I’m trying to send the return package...
- Cannot complete MaltaPost form; missing last name, address, VAT number, and phone.

Im trying to send the return package but I do not have the information to enter on MaltaPost form to declare the package to get the code for the post office to proceed with sending my package.
It asks me for last name, address, Vat #, phone number and so on. I was given this address in Poland where to send it to.

I received an especially escada perfume which was damaged and the box was even very wet and smelling perfume. The perfume costed me 250 zloty which is very expensive. Fragile products are a problem.
